ESA Letter for Grief & Bereavement

Prolonged grief disorder was added to DSM-5-TR in 2022 as a formal clinical diagnosis, reflecting decades of research showing that a subset of bereaved individuals experience grief that does not follow the normal adaptive trajectory. Approximately 7–10% of bereaved adults develop prolonged grief disorder, characterized by persistent yearning, difficulty accepting the loss, bitterness, numbness, and feeling that life is meaningless - symptoms that substantially impair daily functioning well beyond the acute grief period.

ESAs are particularly relevant to prolonged grief for two reasons. First, grief is an inherently social experience - the loss of a significant attachment figure disrupts the attachment system itself. An ESA can provide a new, stable attachment bond that begins to address the attachment disruption at the core of prolonged grief. Second, the daily structure that animal care requires - feeding, walking, attention - counters the functional withdrawal and collapse of daily routines that characterize prolonged grief disorder. This behavioral activation component mirrors what CBT for grief disorder explicitly targets in therapy.

A note on pet loss: bereavement following the death of a companion animal is increasingly recognized as clinically significant. While the FHA's ESA qualification pathway centers on the human's mental health condition, people who have lost an ESA may experience grief that qualifies as an independent mental health condition (adjustment disorder or prolonged grief) for which a new ESA could be therapeutically indicated. A licensed clinician evaluates the full clinical picture regardless of the loss type.
