ESA Letter for Eating Disorders

Eating disorders affect approximately 9% of the US population over a lifetime and have among the highest mortality rates of all mental health conditions - anorexia nervosa has a mortality rate of approximately 10% over 10 years. Despite their severity, eating disorders are frequently underdiagnosed, and many people living with them do not receive adequate specialized care. The psychological component of eating disorders - distorted body image, perfectionism, shame, anxiety, and depression - intersects significantly with the conditions that most commonly qualify for ESA documentation.

The evidence for animal-assisted interventions in eating disorder treatment is emerging. A 2019 pilot study found that dog-assisted therapy sessions reduced anxiety and improved patients' sense of safety during difficult therapeutic discussions about body image and eating behaviors. The proposed mechanisms include disruption of hyper-self-focused attention (difficult to maintain while a dog is demanding engagement), activation of oxytocin-mediated reward pathways that do not involve food, and provision of a non-evaluative relational bond.

Housing stability is a clinical concern in eating disorder treatment because the home eating environment significantly affects recovery. ESA documentation ensures that the animal - whether it helps with mealtime companionship, motivational routine, or simply providing a reason to get up and engage with the day - can remain in any rental. The FHA's ESA protections apply to all residential rentals regardless of pet policies, giving people in eating disorder recovery reliable access to their animal's support throughout treatment.
